Before you begin...

Your Customer will receive a loan decision for FREE and if approved, their approval is valid for 90 days. If your customer
chooses to secure their mortgage with us, they will be asked to provide their credit card information for verification
purposes. No advance fees will be collected from your customer prior to closing unless they decide to cancel their loan
application.If your customer cancels their loan application prior to closing, a NON-REFUNDABLE fee of .5% of the
requested loan amount will be charged to the credit card account that your customer provides as permitted by state
law. If your customer's loan application is declined for any reason, a Fee of $350.00 or actual costs incurred,
whichever is less, is NON-REFUNDABLE and will be retained by us to cover the costs associated with processing your
customer's loan application. The fee will be charged to the credit card that your customer provides.

By checking below, I certify that I completed the steps below and delivered the following disclosures, as well as any other disclosures required to be given by me under federal and state law. Present all required Federal and State Agreements/Disclosures (State Disclosure)
Present a Privacy Pledge (Your company should provide this to you)
Present a Pre-Application Good Faith Estimate/Third Party Service Provider Disclosure (Federal Disclosure)
